VISTA's risk registers are in spreadssheets with the following column
headings

owner
summary of risk
details of risk
progress
area
mitigation strategy
impact (scale 1-5)
likelihood (scale 1-5)
exposure = impacct* likelihhod

only risks with exposure >= 10 are actively discussed
at present.

We may as well do the same
